Please try to complete this form after booking your appointment, it will save us time on the day. This is required for all appointments. If you are unable to do this, we will also bring a form to your appointment.
Before any ear examination or procedure, we will always ask for your consent. This means making sure you understand what the procedure involves, why it is recommended, and any potential risks, so you can make an informed decision about whether to proceed.
Verbal consent is sufficient for an initial otoscopy and for taking ear impressions.
If ear cleaning is required, you will be asked to sign a short consent form.
You are free to ask questions at any time, and you can change your mind or withdraw your consent at any stage.
For anyone under 16 years of age, a parent or legal guardian must be present throughout the appointment, provide consent on the child’s behalf, and sign the consent form. We will also explain the procedure in an age-appropriate way and ensure the child is comfortable and willing to continue. Consent is a shared decision, and we respect each child’s right to choose. If a child does not wish to proceed, the appointment will not continue.
